Instructions for Use:

  1. Participant Name:
    Write the full name of each participant in the course.
  2. Session Date:
    Record the date of each training session.
  3. Session Title/Topic:
    List the title or topic of the session. For example, “Pipe Installation” or “Plumbing Safety Protocols.”
  4. Present (P) / Absent (A) / Late (L):
    Mark “P” if the participant is present for the entire session, “A” for absence, or “L” for late arrival.
  5. Late Time:
    If the participant is marked as “Late,” specify the time they arrived after the session began.
  6. Reason for Absence or Late Arrival:
    If a participant is absent or late, note the reason (e.g., “Medical issue,” “Personal reasons,” “Traffic delay”).
  7. Instructor’s Notes:
    Use this section for any additional comments about the participant’s performance or attendance, such as “made up missed work” or “improved participation in session 3.”
