SayPro 📋 Documents Required from Employee (Upload via SayPro Website): Policy Contribution Log Sheet
- Policy Contribution Log Sheet: All employees involved in policy development, review, or consultation must complete and upload a Policy Contribution Log Sheet to formally record their inputs and participation in SayPro’s policy processes.
- Purpose of the Log Sheet:
- To document individual contributions to policy drafting, editing, feedback, or review activities.
- To ensure transparency, accountability, and accurate recognition of each employee’s role in policy development.
- To support internal reporting, performance tracking, and compliance with SayPro’s documentation standards.
- Log Sheet Requirements:
- Use the official SayPro template for the Policy Contribution Log Sheet (available on the SayPro portal or from your department lead).
- Ensure the log includes:
- Employee full name and department
- Date(s) of contribution
- Policy title or reference number
- Type of contribution (e.g., drafting, reviewing, providing subject-matter feedback)
- Brief description of the contribution
- Employee signature and date of completion
- The completed sheet must be saved as a PDF and must be clearly legible.
- Submission Instructions:
- Log into the SayPro website using your employee credentials.
- Navigate to the “Document Upload” section and select “Policy Contribution Log Sheet” from the dropdown.
- Upload your completed log sheet and name the file using the format:
[FirstNameLastName_PolicyLogSheet] (e.g., AnnaMoyo_PolicyLogSheet.pdf).
- Submission Timeline:
- Submit your log sheet monthly, or as specified by your team lead or project coordinator.
- Ensure your contributions are logged within 5 working days of completing policy-related activities.
