Objective: Conduct a mid-quarter survey to gather feedback on the mindfulness program’s impact and gather suggestions for improvement. Analyze participation rates and make adjustments to the program if necessary.
1. Conducting the Survey
- Survey Design: Develop a comprehensive survey to gather feedback on the mindfulness program. Include questions about the effectiveness of the sessions, the relevance of the content, and the overall impact on employee well-being and productivity.
- Anonymous Responses: Ensure the survey allows for anonymous responses to encourage honest and open feedback. Assure employees that their input is confidential and will be used to improve the program.
- Distribution: Distribute the survey to all participants via email, the company intranet, or internal messaging platforms. Provide clear instructions on how to complete the survey and the deadline for submission.
2. Gathering Feedback
- Feedback Questions: Include a mix of quantitative and qualitative questions in the survey. Examples of questions to ask:
