SayPro Signed Participation & Liability Agreement
All participants must submit a signed Participation & Liability Agreement before taking part in any SayPro program, event, or activity. This agreement is a mandatory document that outlines the expectations, responsibilities, and legal terms related to your involvement.
Purpose of the Agreement:
- To confirm that the participant voluntarily agrees to take part in SayPro activities.
- To acknowledge and accept the potential risks associated with participation.
- To release SayPro and its affiliates from liability for any injuries, losses, or damages that may occur during the program.
- To affirm the participant’s understanding and acceptance of SayPro’s terms, code of conduct, and privacy policy.
Who Must Sign:
- All individual participants (18 years or older).
- Parents or legal guardians of participants under 18, if youth-specific events are offered.
- Authorized representatives for group or company entries.
How to Submit:
- Download the form from the SayPro event or registration page.
- Read thoroughly, complete all sections, and sign where indicated.
- Upload the signed copy during online registration or email it to the SayPro administration team.
Important Notes:
- Participation is not permitted without a signed agreement on file.
- Digital signatures are accepted if submitted through verified channels.
- A copy of the agreement will be provided to all signatories for their records.
